This is a MYTH that has been going around twitter lately. But I see your confusion, let me clear it up
James Vaughan went to Crystal Palace on 'emergency loan' only. That doesn't count in the FIFA rule you are referring to
An emergency loan happens OUTSIDE a transfer window and is limited to a max of 3 months. It is exempt from the rule (a.k.a. a loophole)
So even though he has played for both clubs, he can still play for 1 more this season. Simples.
This isn't true, Mike Grella (Leeds player) loan transfer to motherwell was cancelled today, a day after anouncing his signing. He made 2 subs appearances for Leeds before an emergency loan to Carlisle.

If Tottenham follow up their initial interest in Scott Brown then Lennon will ask for £2-3m and Jamie O'Hara in the deal. I personally would ask for the young lad Sandro on loan for the season (and maybe next) to be included as looks likely that Crosas won't be around come the end of the season.
